The following is an outline of the Golden Wear Clothing Inc. Personal Information Policy which is in harmony with the requirements of the Personal Information Protection and Electronic Documents Act.This policy is effective January 2004. Section A. What is Personal information
For purposes of this policy, Personal Information is defined as: information about an identifiable individual such as e-mail address, credit information, purchase,and; order history, and home address. It also includes information such as: name of the customer, clothing size, medical condition, account and /or client numbers.
Other information specifically related to an individual and or their family would also be included in this definition.
Section B. Definition of Alternate decision Maker
Alternate Decision Maker (ADM)as defined for the purpose of the policy includes persons appointed by you, your family or by the court to disclose your personal information.We will only communicate to you or your ADM unless directed by you or your ADM.
Section C. Collection of Information
We collect personal Information primarily for the purposes of providing products or services. At the time our services are engaged, the following personal information may be collected:
- Name, phone number, home address, e-mail address, trust account information,height,weight,body measurements and other client information that may be needed to complete the purchase and provide our service.
If you send or receive email, fax, phone, catalogue or paper based messages to or from us, we will collect those communications for future reference. This is done to facilitate customer service.
If you place an order on the Golden Wear Clothing Website, the Website will archive personal information such as :name and address for future reference. Financial information, such as your credit card number is deleted once the order is processed.
Section D. Use and Maintenance of Information
Information collected about you will be used specifically to complete the service that you or your ADM requested. If you have purchased an article of clothing or service from us, we will maintain certain personal information for up to seven (7) years, or as required by law for such things as tax purposes.
Golden Wear Clothing Inc. will document and archive your information to facilitate our ongoing business relationship. At the time that you request to be removed from our files your personal information will be removed from our client file permanently (except for any personal information which we are required, by law or for tax purposes to maintain).
Golden Wear Clothing will use the personal information collected about you for the purpose for which it is originally collected, and to facilitate our ongoing business relationship. Examples of this would be: advertising of sales, specials and educating you about new products that are available.
If we identify a new purpose for which your personal information could be used, we will obtain your consent before using your personal information for that purpose. We do not sell any individual's personal information to third parties.
We may collect, use or disclose personal information without an individual's knowledge or consent in circumstances where such collection, use or disclosure is permitted or as required by law.
Golden Wear is dedicated to keeping our records up to date. As long as we have a business relationship we will periodically verify that our records are accurate with you. This verification will take place either in person,or by telephone or e-mail.

Section E. Access to Information
You or your ADM have the right to access the information that we collect about you. You or your ADM may request that you be given access to view the information, and recommend to us that any correction be made.
After receiving a written request we will contact you with in thirty (30) business days to arrange a method for providing you with the access you have requested. In most cases we will be able to provide you with the information by telephone if we can verify your identity, other wise you may be required to attend at our store, at which time our staff will assist you with the requested information.
Please note that we may not be able to provide Personal Information about you if it(i) would reveal Personal Information about a third party,(ii)is subject to legal privilege,(iii) contains other confidential information which would be revealed,(iv)is information that was generated in the course of a formal dispute resolution process,(v) relates to an investigation of a breach of agreement or contravention of laws,or (vi) cannot be disclosed for other legal or security reasons.
If you believe that there is an error in the information on file about you, you can request that our privacy officer review and correct the information as necessary.
There is no fee for this service. To request access to your information; please contact the privacy officer at privacy@goldenwearclothing.com, or by contacting them at the address contained at the bottom of this document.
Section F. Nondisclosure of Personal Information
Golden Wear Clothing Inc. will not disclose your personal information to anyone or any organization without explicit permission from you, or Alternative Decision Maker unless we are required by law.In a specific situation where Golden Wear Clothing Inc. has been asked to share information with another individual a request in writing,will be required. The written request will be kept on record for the purpose it is intended.
If you or your ADM have any questions or concerns about the disclosure of your personal information or beleive that your personal information has been improperly disclosed to another organization, contact our privacy officer at privacy@goldenwearclothing.com.
Section G. Security
Golden Wear Clothing makes all reasonable efforts to protect your information.
Much of the information that we collect about you will be stored by computer, zip drive, paper copy in filing cabinets or other technological methods.
Our customer database is not on a computer that has e-mail access. Our Website is hosted by Clic.net communications, which has encrypted technology to protect personal and financial information about our clients. Our Website is also protected with passwords. Sensitive records such as client numbers, addresses and invoice information is kept secure in our store office.
